CenterLight Healthcare PACE has an approved contract with the Centers for Medicare and Medicaid Services (CMS) and NY State. CenterLight Healthcare Select has a Medicaid partial capitation long term care plan contract with NY State. CenterLight Healthcare Direct is a health plan with a Medicare contract. Depending upon which CenterLight Healthcare program is right for you, additional benefits and restrictions may apply. Members may be liable for the cost of services not authorized by your CenterLight Healthcare program.
