PACE Model of Care Draws International Attention

Social service professionals are coming from as far as Israel and Singapore to see what they can learn about CCM's Program of All-Inclusive Care for the Elderly (PACE).

Israel's director general of the Israeli Ministry of Social Welfare and directors involved with aging and community relations issues visited the Allerton CCM site on September 22 to see how the PACE model addresses the issue of service coordination and integration. Singapore's minister of state and two senior staff members took a similar tour of the Grand Street CCM in Manhattan on Sept. 19 and another Singapore contingent will be coming in October.

Beth Abraham Family of Health Services' PACE program - the largest in the nation - integrates medical care, social day care, home care and supportive care for the frail elderly who want an alternative to a nursing home. The Israeli delegation specifically expressed interest in learning more about how the model is coordinated and integrated to ensure seamless delivery of services to a socially complex and aging population.

Comprehensive Care Management Welcomes Four Diverse and Distinguished Industry Leaders to its Board

Financial, media and marketing, software and technology lions add their expertise and counsel to the CCM Board

BRONX, NY, April 13, 2011 -- Four leaders in the financial, media and marketing, software and technology realms are bringing their collective expertise and counsel to the Board of Directors of CCM, a member of the Beth Abraham Family of Health Services (BAFHS).

The four newly appointed outside board members are Parker Global Strategies Senior Vice President Rick T. Babich, Newspaper National Network’s former Senior Vice President of Media and Operations Gerard A. Fragetti, Microsoft Health Solutions Group’s Howard L. Krain and J. David Nelson, Vice Chairman of the Chicago-based software company, VaporStream, Inc.

Rick T. Babich is the senior vice president of research at Parker Global Strategies, an alternative investment firm in Stamford, CT. Primarily responsible for sourcing, analyzing and recommending hedge fund managers, monitoring investment portfolios and managing a team of analysts, Babich is also a member of the senior management team and chair of the company’s portfolio review committee. Prior to joining Parker Global Strategies in 2008, Babich was a research analyst with Commerzbank's Alternative Investment Strategies group in New York. Gerard A. Fragetti is the president of Max Media LLC, a media consulting company. He is the former senior vice president of media and operations at Newspaper National Network, NY, a $150-million national digital and traditional advertising sales network representing the newspaper industry (and owned by the nation’s top 25 newspaper chains). Previously, Fragetti served as chief operating officer and chief financial officer at Internet Tradeline, Inc.; and Talk Media, both in New York. In 1974, Fragetti was recruited by Rupert Murdoch and joined News Corporation, where he served for more than two decades in key leadership positions including executive vice president of News America’s Free Standing Insert (FSI) and In-Store divisions; chief financial officer of Murdoch Magazines, The New York Post and Skyband, a direct broadcast satellite company. Howard L. Krain is a sales executive at Microsoft’s Health Solutions Group, an innovative business unit that is focused on addressing the challenges of healthcare providers, payers, consumers and life science companies around the world with powerful software solutions that enable lower costs, higher quality care and ultimately better health outcomes. Krain’s focus is to identify and manage strategic partnerships with healthcare providers.. Prior to joining Microsoft, Krain was a Managing Principal and Senior Vice President for the National Healthcare Group within the corporate and institutional banking unit at PNC Financial Services. He has also held senior leadership marketing and business development positions in the healthcare IT and technology units of General Electric. Krain is a committee chairperson at Healthcare Financial Management Association (HFMA), New Jersey Chapter, and is board certified in healthcare management as an ACHE Fellow (FACHE). J. David Nelson served for more than three decades in a variety of management roles with IBM. One of the first executives in what is now IBM Global Services, Nelson’s focus was on new products, new markets and services both in White Plains and Beijing, China (where he managed the company’s largest business unit). After capping his career with IBM, Nelson became chief operating officer of The National Foundation for Teaching Entrepreneurship (NFTE), a New York City-based organization that provides entrepreneurship education programs to young people from low-income communities. He is presently the vice chairman and a board member of VaporStream, Inc., a Chicago-based software startup company, and sits on several boards and committees, including the AARP Foundation and the AARP National Board, and the World Affairs Forum of Stamford, a position he has held for six years. CCM and GCCA Launch Collaborative Relationship with Holiday Celebration

Magic, Music and Santa to visit Comprehensive Care Management site on Chrystie Street

Comprehensive Care Management Corp. (CCM), a leading provider of healthcare services to adults throughout the five boroughs of New York, Long Island and Westchester County, and the Greater Chinatown Community Association (GCCA), a well-established organization devoted to providing support to lower Manhattan’s growing Chinese population, are co-hosting a holiday celebration on Saturday, December 11 from 10a.m. to 1p.m. at CCM’s community day health center at 185 Chrystie Street in Manhattan.

“CCM and GCCA share a strong interest in serving the needs of the residents of Chinatown,” says Matt Lippitt, CCM Vice President of Business Development. “CCM’s two community health centers, on Grand Street and Chrystie Street, already serve more than 400 Chinese members. Our relationship with GCCA will allow CCM to make valuable and innovative contributions to a community we’ve served for many years.”

The holiday celebration is designed to appeal both to seniors and their grandchildren. Featured performers include an eight member troupe of Chinese entertainers who will perform traditional dancing, singing and instrumental music, and a magician who will dazzle the audience with an assortment of skillful tricks. Santa is expected to appear bringing good cheer and gifts for the kids. CCM and GCCA will have a raffle and refreshments for the guests.

“We are dedicated to growing our organization so that we may better meet the complex and varied needs of the Chinatown community,” says Chih-Ping (Andy) Yu, Executive Director of the GCCA. “In addition to a holiday celebration, our partnership with CCM allows us to expand the programs we offer to members. We will be using CCM’s Chrystie Street site for community activities that include an after-school program and ESL classes and its other Chinatown center on Grand Street for Saturday SAT classes for high school students.”

Seniors Receive Access to Personalized Health Care and Therapeutic Recreation

Staten Island, NY, June 2, 2010 – Comprehensive Care Management (CCM), a member of Beth Abraham Family of Health Services, announces its first Program of All-Inclusive Care for the Elderly (PACE) on Staten Island, NY, with the opening of its day health care center on 1106 Bay Street. CCM Staten Island offers seniors a great opportunity to interact with each other in order to prevent isolation and depression. At the 2,100 sq. ft. center, seniors will receive meals and engage in a variety of therapeutic recreation activities, such as singing and instrumental music programs, arts and crafts, and modified exercise. Culturally sensitive to the needs of its diverse member population, CCM Staten Island has staff members who speak languages such as Russian, Spanish, Italian and English.

CCM Staten Island members will receive medical care at the CCM Stillwell site in nearby Brooklyn, NY, through PACE. Round trip transportation will be provided from members’ homes to both sites. CCM is the nation’s largest PACE provider, and offers personalized health care for seniors with long-term medical needs by offering them access to a team of doctors, nurses, social workers, physical and occupational therapists, among others.

CCM has been providing health care to seniors in New York for 18 years, and currently tends to the medical needs of over 4,600 people.

Yonkers, NY, November 19, 2009–Comprehensive Care Management (CCM) Greenburgh announces the grand opening of its day health care center for the elderly in Yonkers, NY. CCM Yonkers offers seniors a great opportunity to interact with each other in order to prevent isolation and depression. At the Center seniors engage in a variety of therapeutic recreation activities, such as tai chi, yoga, dominoes, crocheting, and modified sports and exercise. People from diverse backgrounds and nationalities can often be found working together in groups to create and complete a myriad of projects, including murals. Culturally sensitive to the needs of its diverse member population, CCM Yonkers has staff members who speak English, Spanish and Korean.

CCM members who visit the Yonkers site receive medical care at the CCM Greenburgh site through the Program of All-Inclusive Care for the Elderly (PACE). Round trip transportation to and from both sites is provided to these members. CCM is the nation’s largest PACE provider, and offers personalized health care for seniors with long-term medical needs by offering them access to a medical team of doctors, nurses, social workers, physical and occupational therapists, among others.

CCM has been providing health care to seniors in Westchester for 13 years, tending to the medical needs of over 500 people.

Day Health Center Provides Personalized Health Care for Culturally Diverse Population of Seniors

Brooklyn, NY, October 29, 2009–Comprehensive Care Management (CCM) announces the grand opening of its Program of All-Inclusive Care for the Elderly (PACE) Center in Brooklyn, NY. The PACE program provides personalized health care for seniors in the Brownsville and East New York communities with long-term needs by offering them on-site access to a medical team of doctors, nurses, social workers, physical and occupational therapists, among others. In addition to addressing their medical needs, CCM PACE offers members an opportunity to socialize by participating in an array of therapeutic recreational activities, such as Tai Chi, yoga, fine arts instruction, skilled beadwork, and Wii games. There is also an English as a Second Language (ESL) program for Creole-speaking members who would like to learn English.

Culturally sensitive to the needs of its diverse member population, this CCM PACE center has staff members who speak Creole, Spanish, Cantonese, Mandarin, and other Asian dialects.

Day Health Center in Ridgewood Provides Personalized Health Care for Public

Ridgewood, NY, September 4, 2009–Comprehensive Care Management (CCM) announces the grand opening of its Program of All-Inclusive Care for the Elderly (PACE) Center in Ridgewood, NY. This is the first PACE Center in Queens.

The PACE program provides personalized health care for people with long-term needs by offering them on-site access to a medical team of doctors, nurses, social workers, physical and occupational therapists, among others. In addition to addressing their medical needs, CCM PACE offers members an opportunity to socialize by participating in an array of therapeutic recreational activities, such as origami, modified baseball, puzzles/board games and multi-ethnic celebrations.

“Having on-site medical care and therapeutic recreation in one facility is a huge benefit for the elderly,” said CCM Site Director Karen Humphrey. “From doctors and nurses to stimulating social activities and deliciously prepared food, people from the community have everything they need under one roof.”

Day Health Center Provides Care for Nuns as Well as Public

AMITYVILLE, NY, June 25–Comprehensive Care Management (CCM) announces the grand opening of its Program for All-Inclusive Care for the Elderly (PACE) Center inside the Motherhouse of the Sisters of St. Dominic convent in Amityville, NY. CCM, a member of the Beth Abraham Family of Health Services, is the first company on Long Island that offers a PACE program on-site in a religious congregation. There is also a separate entrance that allows members of the community access to the facility. Having the CCM PACE program on the premises allows the Sisters to maintain their routines, including daily prayer, while receiving personalized medical care and participating in the activities of their choice.

The CCM PACE program in the Sisters of St. Dominic Motherhouse was created to address the needs of the aging population of Sisters that reside there, and is also open to members of the community. The program serves people with long-term healthcare needs by providing them on-site access to a medical team of doctors, nurses, social workers, physical and occupational therapists, among others.

Before CCM, the nation’s largest PACE provider, was established in the Motherhouse, the Sisters received care provided by nurses and aides they hired, and their family members had to arrange for transportation for all off-site doctor visits. Now, with primary care physicians providing quality care on-site daily at the CCM PACE center, if members need to see a specialist, such as a cardiologist or dermatologist, off-site, the program provides an ambulette and a health aide, if necessary, to take members to and from appointments. Also, many of the same nurses and community doctors were retained by CCM PACE in order to ease the Sisters’ transition into the program.

“My favorite thing about CCM is the continuity of care and a greater sense of organization,” said Sister Rose Michael Hillery. “The CCM PACE people take care of us very well.”

“With approximately 43 Sisters currently participating in the program, having on-site medical care is a huge advantage,” said CCM Site Director Kathleen Kadel. “The Sisters and other registrants from the community have everything they need here, without going to a nursing home.”

In addition to addressing their medical needs, CCM PACE offers members an opportunity to socialize by participating in an array of therapeutic recreational activities, such as Art as Meditation, Tai Chi, book club and ice cream socials. According to Community Health Nurse Debra Pfeifer, “With the site open to the public, it gives the Sisters a chance to interact with the community, which is something they enjoy.”

“Since the CCM PACE program began, socialization among the Sisters has increased,” said Day Health Center Manager Carol Hartman. “They continue to enjoy participating in the therapeutic recreational program and have been getting out of their rooms more often.”
