CenterLight Healthcare Direct offers four health plans with a Medicare contract designed to meet your medical and prescription drug needs. To find out which of our plans would work best for you, please see our eligibility comparison chart below.

Special Needs Plans
Complete	Total
Individuals who are entitled to Part A (hospital) of Medicare and are also enrolled in Medicare Part B (doctor) and continue to pay Medicare Part B premium	Individuals who are entitled to Medicare Part A (hospital) and enrolled in Medicare Part B (doctor)
	Have full Medicaid and are eligible for the New York State Medicaid Advantage Plus (MAP) Program
For individuals who require a nursing home level of care whether they reside in a nursing home, assisted living facility or in their own home and community.	For individuals who require a nursing home level of care in their own home and community.
*Live in our service area: Bronx, Kings (Brooklyn), New York (Manhattan), Queens, Richmond (Staten Island), Nassau, Suffolk and Westchester counties
Individuals with End Stage Renal Disease generally are not eligible to enroll unless they have been members since before their dialysis began.
