CenterLight Healthcare Direct Complete Plan
CenterLight Healthcare Direct Complete (HMO SNP) is a Special Needs Plan for Medicare beneficiaries who require a nursing home level of care whether they reside in a nursing home, assisted living facility or in their own home and community.
Nursing home residents are provided with a Nurse Practitioner who, in addition to your physician, visits you often and helps to assess your condition, coordinate your care and keep your loved ones in the loop.
For those still living at home, CenterLight Healthcare Direct Complete Plan will provide a Care Manager who will coordinate all of your care needs. When you enroll, your Care Manager will do a complete health history and develop a plan of care with specific goals dealing with your health concerns. Your Care Manager will also coordinate care between all your providers and act as your advocate in the health care system.
