Where People Come First

When it comes to the issues of aging, we want the best care for ourselves and our loved ones, but getting that care can be frustrating. Trying to get from one doctor’s appointment to another, managing medications or struggling with the activities of daily living is difficult, especially when there is no one there to help or answer your questions.

CenterLight Healthcare ‘s Program of All-Inclusive Care for the Elderly (PACE) enables those whose health issues are interfering with their daily functioning and quality of life to stay safely in their own homes and communities for as long as possible, while receiving high quality, comprehensive care.

At CenterLight Healthcare’s day centers, you can participate in enjoyable recreational activities, make new friends and look forward to nutritious meals. Our day centers also offer a variety of services, including medical care, rehabilitation and other types of therapy and social work services, such as counseling, designed to keep you as healthy as possible. Our team of physicians, nurses, social workers and therapists work with you and your family to develop a healthcare plan that’s right for you. We also provide transportation from your home to our day centers, doctors’ appointments, and other health-related needs.

In addition, our day centers provide needed respite for caregivers to enable them to go to work or attend to other responsibilities.

CenterLight Healthcare PACE also has Alternative Care Settings, which are an option for those PACE participants who prefer a smaller, more intimate setting to receive on-site socialization, social services, personal care services,therapeutic recreation and meals. Other center-based services, such as primary care physician services, nursing and rehabilitation are offered at our PACE Centers.

Members may be liable for the cost of services not authorized by your CenterLight Healthcare program.

Each of our PACE Centers offers its members a wide variety of daily activities, many of them designed to appeal to specific cultural preferences, including:

Exercise such as Tai Chi, Qigong, Yoga and Walking Club

Arts and Crafts

Russian Card Games, Bingo, Dominoes and Table Games

Sewing and Knitting, Calligraphy

Karaoke, Piano Lessons, Irish Song Lessons, Drum Circles

Jingo, Balloon Volleyball, Jenga, Air Hockey Club, Ping Pong, Billiards and Other Games
