Do I Qualify?
Eligibility requirements for CenterLight Healthcare Select include:
Being 18 years old or older
A resident of Bronx, Kings (Brooklyn), New York (Manhattan), Queens, or Westchester county
Medicaid eligible
Nursing home eligible
Able to live safely at home at time of enrollment
Must require long term-care services of the plan for a minimum of 120 days or more
