What is Managed Long Term Care (MLTC)?
Managed Long Term Care (MLTC) is a program designed to help elderly, disabled or chronically ill individuals live safely in their own homes and communities. A MLTC plan arranges and pays for a wide variety of health and social services, not including physicians or hospital care.
Are there different types of Managed Long Term Care?
CenterLight Healthcare offers two basic models of Managed Long Term Care in New York State: Program of All-Inclusive Care for the Elderly (PACE) and Managed Long Term Care (Select).
Program of All-Inclusive Care for the Elderly (PACE). PACE offers a comprehensive system of healthcare services for members age 55 and older who are eligible for nursing home admission, but prefer to remain living safely in their homes and communities. The plan provides PACE members with primary care physicians and a healthcare team, including a home care nurse, social worker, rehab specialists, home health aide/personal care aide, and a transportation representative, that develop care plans and provide on-going care management. The program also provides or arranges all necessary hospital and long-term care services. Both Medicare and Medicaid pay for PACE services.
Managed Long Term Care Plans (Select). Managed Long Term Care plans provide long-term care services such as home health and nursing home care, social day care or adult day healthcare, and other services such as dentistry and medical equipment. Members obtain services from their own primary care physicians and inpatient hospitals using their Medicaid and/or Medicare cards. Members must be eligible for nursing home admission to qualify for CenterLight Healthcare Select.
Am I eligible for Managed Long-Term Care?
Eligibility requirements for CenterLight Healthcare Select Managed Long Term Care include:
Being 18 years old or older
Currently on Medicaid or Medicaid eligible
Living in our service area, including Manhattan, the Bronx, Brooklyn, Queens, Kings or Westchester County
In need of nursing home level of care and able to remain living safely in your community with the services of the plan
Must need long term-care services of the plan for a minimum of 120 days or more.
How do I enroll in Managed Long-Term Care?

How does MLTC affect my Medicaid and/or Medicare coverage?
If you are already receiving services under Medicare, CenterLight Healthcare Select will not affect them at all. There will be changes that affect Medicaid, however, because CenterLight Healthcare Select is a managed care program. That means that you will be able to obtain certain services only from providers in the CenterLight Healthcare network. We would be happy to answer your questions in more detail if you call us.
