CenterLight Healthcare Select Medicaid Managed Long Term Care members benefit from the coordination of care provided by a nurse care manager who works with you, your family and physician to develop an individual plan of care. CenterLight Healthcare Select members maintain their own physicians and hospitals, while receiving other long-term care services through our network of high quality providers. Some services, including dental, vision, medical equipment and transportation, can only be provided within our CenterLight Healthcare Select network.

CenterLight Healthcare Select arranges and pays for a large selection of health and social services. The plan does not cover physicians, hospitals and certain other services. Only services called for in an individual’s plan of care and available in our network will be covered by CenterLight Healthcare Select.

Eligibility requirements for CenterLight Healthcare Select include:

Being 18 years old or older

Currently on Medicaid or Medicaid eligible

Living in our service area, including the Bronx, Kings (Brooklyn), New York (Manhattan), Queens, or Westchester county

In need of nursing home level of care and able to remain living safely in your community with the services of the plan. The individual must require long term-care services of the plan for a minimum of 120 days or more.

Members may be liable for the cost of services not authorized by your CenterLight Healthcare program.
