How to Access Transportation
CenterLight Healthcare Select covers transportation to and from medical appointments or adult social day program activities that are part of our members’ plan of care. If you need to schedule transportation, please follow the guidelines below:
To schedule transportation, call us 24 hours in advance of the appointment at 1-800-964-6548, Monday through Friday, 8:00 a.m. to 5:00 p.m., and we would be happy to assist you with your arrangements.
Please note that as schedules have to be coordinated with our transportation vendors, we advise that you call us before 3:00 p.m. to ensure that all arrangements are made prior to the close of business hours.
If your appointment falls on a Monday, please remember to call us the Friday before, as our Transportation Office is not open on weekends.
Transportation covered may include public transportation, Access-A-Ride, car service, ambulette or ambulance, depending on your needs. Please let us know in advance if you have a wheelchair, need oxygen, or if you have any other special needs.
On the day of your appointment, please bear in mind:
For all medical appointments, you will be picked up one hour in advance. Please be fully dressed and ready at pick-up time.
The driver is only required to wait 10 minutes for you. If you are not ready when the driver comes, please call our Transportation Office at 1-800-964-6548.
The drivers are not allowed to accept any tips.
The driver is not allowed to take you to any location other than your home on the way back from medical appointments or the day program.
If needed, an aide can accompany you provided you have approval from your Nurse Care Manager.
For your safety, please note the following:
The driver will escort you from your apartment door into the ambulette and then into the medical facility or the day care. This process will be repeated on your return trip.
It is important that your safety belt remain fastened at all times while you are in the ambulette.
You are prohibited from changing your seats in the ambulette at any time.
You must immediately report any accidents or injuries to the CenterLight Healthcare Select staff.
