As a member of CenterLight Healthcare Select, a Managed Long Term Care (MLTC) Plan, your CenterLight Healthcare Select Nurse Care Manager will work with you, your family and physician to develop a program that’s just right for you. Your CenterLight Healthcare Select healthcare team will coordinate with your physician to monitor your health and well-being and make adjustments to your care plan as needed -- helping you live safely in your own home and community.
