CenterLight Healthcare is the nation’s largest Program of All-Inclusive Care for the Elderly (PACE) and offers individuals access to all the services and resources they need to remain safely in their homes and communities. Our healthcare team of physicians, nurses, social workers and rehab specialists work with you and your family to develop a care plan tailored to your specific medical, emotional and social needs. Your CenterLight Healthcare team continuously monitors your health and well-being and will make adjustments to your plan of care as needed.

Once enrolled, you will receive all your necessary medical and health services through CenterLight Healthcare. PACE gives members access to primary care physicians, specialists, home health aides, transportation, recreation and nutritious meals at our day centers, as well as medical assistance, as needed. Services include:

Primary and specialty medical care, including dental, podiatry and eye care, either at CenterLight Healthcare’s day centers or in the community

Nursing services at home or at CenterLight Healthcare’s day centers

Rehabilitation services, including physical, occupational, speech and hearing therapies

Nutritional counseling

X-Rays and other laboratory services

Medications

Medical equipment

Hospital care

Transportation to and from our day centers and medical appointments

24/7 emergency medical services

Recreational activities, including social discussion groups, arts and crafts, parties, cultural and education programs at our day centers

Social work services for members and caregivers, including counseling focused on health and adjustment issues, help with benefit entitlements and other assistance you may need

Nutritious meals in our day centers

Aides in the home to assist you with personal care, meals, shopping and other aspects of daily living

Therapeutic home modifications to accommodate your disabilities and special needs

Support for family caregivers

Nursing home care

CenterLight Healthcare members may be liable for the cost of services not authorized by your CenterLight Healthcare program.
