Our Services

CenterLight Healthcare Select, a Medicaid Managed Long Term Care (MLTC) Plan, offers services through our network of highly qualified, caring providers. Our services are designed to help you live safely and comfortably in your own home and community, and depending on your needs may include:

Care management

Nursing home care

Home care (nursing, home health aide, physical therapy, occupational therapy, speech pathology, and medical social services)

Social day care or adult day health care

Personal care

Durable medical equipment, medical/surgical supplies, prosthetics and orthotics

Personal Emergency Response System (PERS)

Transportation to and from your scheduled health-related appointments

Podiatry

Dental care

Optometry exams and eyeglasses

Physical, occupational and speech therapies or other therapies provided outside of your home

Audiology exams, hearing aids and batteries

Respiratory therapy, including preventive, maintenance and rehabilitative airway-related techniques and procedures

Nutrition

Private duty nursing

Home delivered or congregate meals

Environmental support, including home improvements and modifications, appliances and assistive devices

Members’ physicians are required to collaborate with CenterLight Healthcare Select and the member on developing and coordinating a plan of care. Only services included in the plan of care will be covered by CenterLight Healthcare Select.

Non-covered services include physician, hospital and others (click here for complete list), and are paid for by Medicare and/or Medicaid fee-for-service. Members can select providers of their choice for non-covered services.

CenterLight Healthcare has a Medicaid partial capitation managed long term care plan contract with NY State. Additional benefits and restrictions may apply.

Members may be liable for the cost of services not authorized by your CenterLight Healthcare program.
