Which Plan Works For Me?

CenterLight Healthcare Direct has four plans, one of which may be right for you.

Special Needs Plans:

CenterLight Healthcare Direct Complete Plan (HMO SNP) is for Medicare beneficiaries who require a nursing home level of care whether they reside in a nursing home, assisted living facility or in their own home and community. Nursing home residents are provided with a Nurse Practitioner in the nursing home and for those living at home, CenterLight Healthcare Direct Complete will provide a Care Manager who will coordinate all of your care needs.

CenterLight Healthcare Total Plan (HMO SNP) is for Medicare beneficiaries who have full Medicaid/are dual eligible and require a nursing home level of care in their own home. The plan coordinates all of your Medicare and Medicaid benefits, including prescription drug benefits. It also arranges your medical services through CenterLight Healthcare’s extensive network of health care facilities and physicians.
