CenterLight Healthcare's Program of All-Inclusive Care for the Elderly (PACE) enables those who have health issues that are interfering with their functioning and quality of life to stay safely in their own home and community for as long as possible and receive high quality, comprehensive care. In PACE there is never a co-pay, deductible or coverage gap, so you can access the care, services and medications you need.

Our Team Approach

As the largest Program of All-Inclusive Care for the Elderly (PACE) provider in the country, CenterLight Healthcare PACE offers complete medical care, home care, rehabilitation therapy, medical transportation and a no-cost drug benefit.  CenterLight Healthcare PACE will also cover nursing home care if that should become necessary.  Our healthcare team of physicians, nurses, social workers and rehab specialists work with you and your family to develop a care plan tailored to your specific medical, emotional and social needs. The team continuously monitors your health and well-being, making adjustments to your plan of care as needed.

A Caring Place To Go

At CenterLight Healthcare’s day centers you can participate in enjoyable recreational activities, make new friends and look forward to nutritious meals. Our day centers also offer a variety of services including medical care; rehabilitation and other therapies; and social work services, such as counseling, designed to keep you as healthy as possible. We also provide round-trip transportation from your home to our day centers, doctor’s appointments and other health-related services. In addition, our day centers provide needed respite for caregivers to enable them to go to work or attend to other responsibilities.

A Helping Hand at Home

In your own home, CenterLight Healthcare PACE will provide the extra help you need to stay safe and comfortable. We also assist family members and other caregivers with the support they need. In addition to nursing care at home you may also receive trained staff to help with personal care, shopping, meal preparation, simple household needs, and even modifications to your home to accommodate any disabilities or special needs. 

Members may be liable for the cost of services not authorized by your CenterLight Healthcare program.
