CenterLight Healthcare’s PACE Program (Program of All-inclusive Care for the Elderly), the largest program in the country, provides a full range of care your patients need to continue living safely at home.
All primary care providers have patients in their practice that fit the typical PACE patient profile. These patients have multiple medical problems, require multiple referrals for the non-medical care needed to keep them in the community and often require frequent conversations/meetings with family members and caregivers to coordinate their care.
Better use of your time, appropriate compensation for it
CenterLight Healthcare’s PACE program can help. Serving as both the health plan and the provider of non-medical care through one point of contact with the PACE Community Health Nurse, PACE covers the full spectrum of services your patient needs to live a healthier, more independent life. In addition, you will be compensated with an annual case rate that is higher than per-visit fees in recognition of the additional time you will spend during each office visit focusing on your patient’s medical needs, and working with an interdisciplinary team of care professionals.
Your patient remains your patient
If your patient is accepted into the CenterLight PACE program, you remain the primary physician in accordance with your patient’s wishes. His or her non-medical care becomes the responsibility of the PACE Interdisciplinary Team (IDT) of nurses, therapists, social workers, behavioral health therapists and others through one point of contact for you.
- Team members mirror the cultural and linguistic diversity of your patient.
- IDT monitors your patient's health and prevents case escalation, resulting in lower hospitalization rates.
- A nurse, social worker, physical therapist and other members of the team are available every day at PACE sites.
Services provided and coordinated by the IDT include:
- Rehabilitation services
- Social services
- Behavioral health services
- Socialization with peers
- Medication (no copays)
- Medical equipment
- Recreational therapy
- Dental care
- Home care services (in-home aids)
- Medicaid recertifications
The PACE program also covers and coordinates all the supporting non-primary medical care your patients may need:
- Other physicians/specialists
- Hospital care
- 24 hour emergency medical services
- Laboratory and radiological services
To see if your patient is eligible, at no cost, call 1-877-226-8500. Patients who do not qualify for Medicaid but can pay privately are able to join PACE if they meet the remaining eligibility criteria above.
