CenterLight Teamcare is a unique managed long-term care insurance program called PACE. PACE is a national Program of All-Inclusive Care for the Elderly that maximizes Medicare and Medicaid benefits by providing on-site medical, rehabilitation and recreation services available in your home and at our all-inclusive centers. At the center, they can have the convenience of seeing the doctor without having to travel to another location, get unlimited rehab therapies, use the open gym, do fun activities and so much more!

The CenterLight TeamCare™ Interdisciplinary Team is a group of dedicated healthcare professionals who work together with one goal in mind: to provide you with the care you need when you need it.

Your care is better when you have a team working and talking together. The Interdisciplinary Team includes, but is not limited to, the individuals below. Click to the right of each icon to find out how each team member can help support your health and wellbeing.

 

Therapeutic Recreation Specialist

My job is to make sure you have fun and make new friends while at our centers! I ask what you like to do, find out your interests, and see if you’d like to learn any new skills. I help you meet other people with similar interests to participate in games, exercises and other wellness activities. Because I talk with others who help with your care, I can structure activities and games to help with your therapy. If needed, I can also visit you at your home to help finds ways to keep you active.


Registered Dietitian Nutritionist

I help you feel better and have more energy, enjoying the foods you like. I can give you advice on how to achieve a healthy and comfortable body weight. If your doctor gives you a special meal plan to follow, I can help you pick the right meals. I look at your overall health and determine if certain foods can help prevent conditions from worsening (like diabetes or high cholesterol). I’m frequently at the PACE centers, can visit you at your home, and available to talk with your aide or your family


Occupational Therapist

My job is to help you stay as independent as possible with your own personal care. I help you achieve the small steps to continue doing routine activities. I can help reteach you how to button your shirt or zip up your jacket. If you have trouble combing your hair or brushing your teeth, I help you with that, too. Sometimes you might need help cooking a meal or feeding yourself—I can work with you. I assess your home to see if it can be made safer, including helping you get needed devices like bathtub benches or bars.


Community Health Nurse

I make sure all of your healthcare needs are being met through regular checkups and assessments. I visit with you once a month (or more often if needed!) wherever you need me—in your home, at a PACE center or even in the hospital. I teach you, your family and your aide how to stay your healthiest based on your condition. If you need it, I help you get care from a doctor, social worker, nutritionist, physical therapist or others. As your nurse, I enjoy being your central point of contact and coordinator of your home care and personal care services.


Physical Therapist

If you got sick or injured, my goal is to help you regain your ability to move or walk as you did before! It is important to me that you have as little pain and discomfort as possible, so I give you exercises and movements to help you recover. Sometimes disabilities are unavoidable, but I can help you learn how to make adjustments to keep you as independent as possible. I like to structure my care through games, which can help you stay flexible, gain strength and maintain good balance. You can find me in the PACE center, but I can come to your home if needed!


Primary Care Physician

I give you check-ups, vaccinations, and ask you how you are feeling on a regular basis! I set goals to help you manage your medical care and to prevent conditions from worsening. I make sure you are given needed prescriptions, see the right specialists, and keep you from going to the hospital unless necessary. I’m different from other doctors because at the PACE center I only work with you, the PACE participant. I’m located at every PACE center, and available to talk with your family, specialists, your aide or other healthcare professionals.


Speech Therapist

My goal is to help you maintain or regain your ability to communicate effectively. If your speech needs to stay limited, I help you find new ways to communicate (such as visual cues or speaking slowly). If you have difficulty swallowing food or water, I can help you find the right drinking cups, foods and even the best position to be in when eating. Depending on your need, I might use tablets, picture boards, singing or other methods to help. I’m frequently at the PACE centers, can visit you at your home, and available to talk with your aide or your family.


Social Worker

I’m here to help you adjust to any changes you may have to make in your life because of your health or treatment plan. You can bring me your paperwork for bills, insurance or anything else you receive if you want to talk about their meaning and impact. I can help you understand the benefits and entitlements you can receive, and then help you apply for them. I can determine if you qualify for assistance with rent payments, food vouchers, utility payments, housing, and much more. I’m happy to speak with your family and caregivers, and can visit you in your home!


Transportation Coordinator & Driver

I help you get places where you couldn’t go before because of your condition. I come right to your front door, and escort you to your appointments safely. I learn your schedule and help you stick to it! I get to you know you and work with you on a regular basis, and let you control the radio station! I’m also available to speak with your family members or home health aide if needed. I am often the last and first person to see you in the day, so I can alert my team members if you need any attention.


PACE Center Manager

My goal is to make your overall experience at our PACE centers enjoyable, fun and beneficial! Your safety and well-being is my highest priority when you visit. I oversee the many services that you get and work closely with our team to ensure it’s provided at the highest quality. I enjoy getting to know each of you and listening to any of your concerns, questions or anything else on your mind! I’m at the PACE center regularly and am available to speak with you, your family or anyone else if needed.

If you are new to Managed Long Term Care, we will refer you to the Conflict-Free Evaluation and Enrollment Center (CFEEC), phone number 1-855-222-8350*.

*TTY users should call 711.

If you are transferring:

  • We will send a nurse to meet with you and your caregiver to discuss your care needs and long-term options in detail.
  • If necessary, we’ll help you navigate the government approval process to expedite coverage and services.
  • We’ll formulate an individual care plan designed to meet your specific needs. You may also contact us by filling in the form on above.
  • Maximus (New York Medicaid Choice), in cooperation with CFEEC, will then conduct an evaluation using a tool called the Uniform Assessment System (UAS) to determine your eligibility for community-based long term care. Individuals who are deemed eligible for enrollment will receive education from CFEEC regarding plans in the area. If you are interested in enrolling into CenterLight, we advise you to inform CFEEC of your preference.
