Specific Coding is Key

The importance of documenting to the highest level of specificity to give a comprehensive and accurate picture of your patient.

CMS requires documentation in the person’s medical record by a qualified health care provider to support the submitted diagnoses. Documentation must support the presence of the condition and indicate the provider’s assessment and/or plan for management of the condition.

This must occur at least once each calendar year for CMS to recognize the individual continues to have the condition.

It is important that the documentation is coded to the highest level of specificity to give a comprehensive and accurate picture of your patient.

Using nonspecific or unspecified codes is not coding to the highest level. Use these codes only if you cannot substantiate a more specific code. An example is chronic ischemic heart disease, unspecified, I25.9. However, if the patient has atherosclerotic heart disease, a native coronary artery with unstable angina pectoris, code I25.110.


Coding Updates
for COVID-19

As the 2019 Novel Coronavirus Disease (COVID-19) continues to impact the medical care we offer to CenterLight participants, we would like to make you aware of updates to coding for COVID-19.  This update is for dates of service October 1, 2020 and forward – Code only a confirmed diagnosis of COVID-19 as documented by the provider or documentation of a positive COVID-19 test results.


ICD 10 UPDATES AS OF DATES OF SERVICE 10/1/20 for COVID-19

ICD-10 Description Notes
U07.1 For a confirmed COVID-19 diagnosis Do not use this code if provider documents “suspected,’ “possible,” or “inclusive” (Instead code for signs and symptoms)
Z20.828 For an asymptomatic individual with actual or suspected exposure to COVID-19 Provider notes indicated contact with and (suspected) exposure to other viral communicable diseases
Z86.19 For patients with a history of COVID-19 Personal history of other infectious and parasitic diseases
Z09 For follow up visits after COVID-19 infection has resolved – For individuals who previously had COVID-19 and are being seen for follow up evaluation and COVID-19 test results are negative Follow up examination after completed treatment for conditions other than malignant neoplasm
Z01.84 For an encounter for antibody testing that is not being performed to confirm a current COVID-19, not as a follow up test after resolution of COVID-19 Encounter for antibody response examination

Highlights from the Provider Appreciation Virtual Gathering:

We would like to thank CAIPA, Expert Medical Services, Ben Sinai Medical Center, Linden Medical, Accent Care, Sachin Kamath MD PC, and CenterLight Medical Staff for attending our first Virtual Provider Appreciation Event on December 10, 2020. 

David Rock, our CEO, welcomed attendees, thanking them for their continued work in caring for CenterLight participants. Dr. Sandra Nieves, CMO, introduced our guest speaker, Michael A. Palese, M.D. Urologist at Mount Sinai Medical Center.  Dr. Palese discussed “Benign Prostatic Hyperplasia and Prostate Cancer” and shared the most up to date diagnostic and treatment information. We’re happy to share the first part of the event for those who weren’t able to attend.

As a thank you to all who participated, CenterLight and Mount Sinai Medical Center will issue CEU(s).

Tiger Connect  

Tiger Connect is the software that CenterLight has been using for its HIPAA-compliant messaging and Facetime calls within the organization. Tiger Touch is a new addition that will allow CenterLight to Facetime with our participants without them having to download any software on their phones.

As Medicare Open Enrollment 2020 closes

Medicare Open Enrollment closed December 7, 2020, locking most Medicare Patients into their chosen plan until the fall of 2021.  However, CenterLight PACE participation may be the exception that meets the needs of Medicare patient(s).   

As a PACE program, enrollment and disenrollment can occur at the discretion of the participant.  This is important to keep in mind as you see your patients thought the year.  Are you noticing a significant change of status in one of your patients?  Has a family member reached out to express concern over the well-being of a loved one?  Consider suggesting CenterLight PACE as an option, we have an Intake Team that would be happy to explain the PACE program in greater detail. 

Let’s work together to provide the best possible care to our patients.
